Fractures of the femoral shaft are one of the most common injuries treated by orthopedic surgeons. These fractures are often associated with polytrauma and can be life-threatening. They commonly result from high-energy mechanisms such as motor vehicle collisions with sequelae of limb shortening and deformities if not treated appropriately. Femoral shaft fractures (FSF) typically occur in a bimodal distribution, with high-energy trauma in young populations and lower-energy trauma in older populations. FSFs are also associated with other comorbidities necessitating a thorough advanced trauma life support assessment and interdisciplinary care. Intramedullary nailing is the most common treatment for physiologically stable patients. The goal of fixation is early healing and long-term functional recovery. Treatment of modern-day femoral shaft fractures results in excellent outcomes.

Methods This register-based study evaluated 2259 patients who underwent revision THA or TKA or any surgery for a femoral fracture between January 2004 and December 2016 at the only hospital in our district where these types of operations are performed. During the study period, the diagnosis and operation codes of the operated-on patients varied greatly, and they were somewhat inaccurate. We thus evaluated radiographs of all 2259 patients one by one, and created inclusion and exclusion criteria based on radiologic findings and medical records. Of those, 12% (279 of 2259) had periprosthetic fractures that met the inclusion criteria, and from these, we formed two study groups (periprosthetic proximal femur fractures, n = 171; periprosthetic distal femur fractures, n = 108). Eighty-eight percent (1980 of 2259) of the patients were excluded because they were treated for a condition other than periprosthetic femoral fracture. The follow-up period ended in December 2019 or at the time the patient died. If the patient presents it’s considered a dx but if unexpected capture as a hospital event. First, If a patient comes in with a temperature of 35 degrees Celsius on admission are we to be coding this as a diagnosis, as it’s an injury in the AIS book that would give you an ISS score? Second, In the below clarification from AAAM it states to not round up or down for a temperature. If a patient has a temperature of 34.7 Celsius on admission is this then a diagnosis or does the temperature have to be 34 or less? What about picking it up as a hospital event 34 or less or can we pickup if the patient has a temperature of 34.2?
